N484 Remark Code (Deactivated): Periodontal Charts Invalid

N484 meant the periodontal charts sent with a dental claim were incomplete or invalid. X12 deactivated it without naming a replacement; the active remark N402 covers incomplete or invalid periodontal charting.

X12 deactivated RARCN484 on May 1, 2015. Payers should no longer use it on new remittances, but it can still appear on older ERAs, corrected claims, and appeals.

What N484 meant

Remark N484 went to dental offices whose periodontal documentation did not pass review. The payer received the charting, but something was missing or not acceptable, so it could not confirm the periodontal treatment was justified.

What replaced it

No successor was named. The active remark N402 says periodontal charting was incomplete or invalid, while N401 covers charting that is missing entirely. At the adjustment level, CO-251 says the documentation received was deficient.

It only appears in older dental data. For a current rejection, review the plan’s charting requirements, update the chart with complete, dated measurements for the teeth involved, and resubmit with any supporting radiographs.
