N483 Remark Code (Deactivated): Periodontal Charts Missing

N483 meant the periodontal charts the dental payer needed were missing. X12 deactivated it without naming a replacement; the active remark N401 covers missing periodontal charting.

X12 deactivated RARCN483 on May 1, 2015. Payers should no longer use it on new remittances, but it can still appear on older ERAs, corrected claims, and appeals.

What N483 meant

Remark N483 was a dental documentation remark. For periodontal procedures, payers review charting that records measurements around the teeth to confirm treatment is warranted. N483 said the charts were not received with the claim.

What replaced it

X12 did not name a successor. The active remark N401 covers missing periodontal charting, and N402 covers charting that was sent but was incomplete or invalid. Dental payers may pair these with CARC CO-252, which says documentation is required.

If you still see N483

It only appears in older dental remittances. For a current request, send a dated periodontal chart with the measurements for the teeth treated, along with any radiographs the plan requires, and reference the claim number.
