What N352 means

Some payers pay for extended treatment on a schedule, sending periodic payments rather than paying visit by visit. N352 is an alert saying this service is not one of them. There is no payment schedule set up, so the payer expects a separate claim for each visit.

Because it starts with “Alert,” N352 doesn’t deny anything by itself. Look at the CARC on the same line to see whether anything was paid or adjusted.

What to do

  1. Read the accompanying CARC. If the claim was denied (for example with CARC 16), that code determines the fix.
  2. If you billed the whole course of treatment up front, resubmit it as individual claims, one per visit date, following the payer’s instructions.
  3. Update your billing setup for this payer so future visits are billed as they happen rather than held for a scheduled payment.
  4. If you believe the payer should have set up scheduled payments, contact provider services and ask what its policy requires.

Do I need to correct anything?

Only if you were relying on scheduled payments. Going forward, bill each visit as its own claim with its own date of service.

What kinds of services get scheduled payments?

Some payers pay long treatment courses, such as orthodontics, in installments. N352 tells you this particular service is not handled that way.

Is N352 a denial?

No. Alerts carry information only. Any denial or reduction on the line comes from the CARC reported with it.
