N290 Remark Code: Missing Rendering Provider NPI

N290 means the rendering provider's primary identifier, their individual NPI, was missing, incomplete, or invalid. It usually appears with CARC 16, and the fix is to verify and correct the rendering NPI, then resubmit.

What N290 means

When a group bills a service, the payer needs to know which individual clinician performed it. That is the rendering provider, identified by a Type 1 NPI in box 24J (or in the rendering provider section of the 837P). N290 means the payer couldn’t use the NPI that was there, or didn’t find one.

Paired with CARC 16, it reads simply as “missing rendering provider NPI.” Payers may also pair it with CARC 206, 207, or 208 to describe the kind of NPI failure.

Common causes

  • A new clinician started seeing patients before their NPI was loaded into the billing system.
  • The group NPI was entered in 24J.
  • The NPI belongs to a different clinician than the one who performed the service, often after schedule swaps.
  • A transposed digit failed the check-digit test.
  • The clinician’s NPI isn’t linked to the group’s enrollment with this payer, so the payer treats it as invalid for the claim.

How to fix it

  1. Confirm from the documentation who performed the service on each denied line.
  2. Verify the clinician’s individual NPI in NPPES.

  4. Correct box 24J (and the 837P rendering fields) on the affected lines.
  5. Resubmit a corrected claim with frequency code 7 and the original claim number.

How to prevent it

  • Don’t schedule a new clinician for a payer until their NPI is loaded and enrollment is confirmed.
  • Validate NPI check digits and entity type automatically.

In the unshaded area of box 24J on each service line of the CMS-1500. On electronic claims it is reported at the claim level or on the specific line when the rendering clinician differs.

Does a solo provider need a rendering NPI?

If you bill under your own individual NPI in box 33a, many payers don't need a separate rendering NPI. Groups billing under a Type 2 NPI almost always do.

Can I use the group NPI in 24J?

No. Box 24J is for the individual clinician who performed the service. A group or organization NPI there is a common cause of N290.
