What MA134 means

Some services to patients living in facilities, such as nursing homes, require the claim to identify the facility where the patient resides. The payer uses that identifier to coordinate with the facility’s own billing, for example to check for services that are part of a facility stay. MA134 says the facility’s provider number was missing, incomplete, or invalid.

It usually accompanies CARC 16.

Common causes

  • The service facility fields were left blank for a nursing facility visit.
  • The billing provider’s number was reported instead of the facility’s.
  • The facility changed ownership and its identifier changed.
  • The patient moved facilities and the old identifier was used.

How to fix it

  1. Confirm where the patient resided on the date of service.
  2. Get the facility’s provider number or NPI from its business office.
  3. Enter it where the payer requires, usually item 32/32a.
  4. Resubmit the corrected claim, using resubmission code 7 with the original claim number in item 22 when replacing a processed claim.

How to prevent it

  • Keep a list of facilities you serve with their current identifiers.
  • Update patient residence information at each visit.
  • A Claims Validator can flag facility-visit claims without a facility identifier.
