N805 Remark Code: Processed Through Correct Code Editor

N805 is an alert that the claim or service was processed through the Correct Code Editor (CCE), a payer's correct-coding edit system. It gives context for any bundling or code-pair adjustment on the same line and requires no action by itself.

What N805 means

Payers screen claims against correct-coding rules to catch code pairs that shouldn’t be billed together, mutually exclusive procedures, and modifiers that don’t fit. N805 says the payer used its Correct Code Editor (CCE) for that screening on your claim.

The alert has no instruction attached. It becomes useful when the same line shows a bundling or code-pair adjustment, because it confirms that a coding edit caused it rather than a coverage or eligibility rule.

What to do

  1. Check the line’s CARC. A paid line with N805 needs nothing further.
  2. If a line was denied or bundled, identify the other service it was paired against and review whether the two were distinct.
  3. If they were distinct and the edit allows it, correct the claim with an appropriate modifier in box 24D and resubmission code 7 in box 22, with documentation to back it up.

  • CO-236 (This procedure or procedure/modifier combination is not compatible with another procedure or procedure/modifier combination provided on the same day…): A procedure or modifier pair was incompatible with another service on the same day.
  • CO-97 (The benefit for this service is included in the payment/allowance for another service/procedure that has already been adjudicated.): The service was included in payment for another service already adjudicated.
  • CO-4 (The procedure code is inconsistent with the modifier used.): The modifier used doesn't fit the procedure code.

N805 FAQ

Do I need to do anything when I see N805?

Only if the line was reduced or denied. The alert explains which system made the decision; the CARC tells you what the decision was.

Is the Correct Code Editor the same as NCCI?

Not necessarily. Payers build their correct-coding editors on their own rules, often drawing on NCCI concepts. Ask the payer which edit set it applies if you need to dispute a result.

Can a modifier override a CCE edit?

Sometimes. If services were truly separate and documentation supports it, an appropriate modifier may allow payment, but only where the edit permits a modifier.
