What N756 means

Ambulance and other transport claims describe a trip with two ends: where the patient was picked up and where they were dropped off. N756 flags the second end. The destination address was left off, incomplete, or failed the payer’s validation.

Without it, the payer cannot confirm the trip, check mileage, or apply rules about which destinations are covered. It usually appears with CARC 16.

Common causes

  • Only a facility name was entered for the destination, with no street address.
  • The ZIP code was missing or did not match the city and state.
  • A PO box or billing address was used instead of the physical location.
  • The crew recorded the destination in the report, but it did not flow into the claim.
  • Multi-leg trips were billed with the wrong leg’s destination.

How to fix it

  1. Check the patient care report for the actual destination.
  2. Enter the complete physical street address, city, state, and ZIP code.
  3. Confirm the mileage billed is consistent with the pick-up and drop-off points.
  4. Submit a corrected claim with resubmission code 7 and the original claim number.

How to prevent it

Keep a validated address list for the hospitals, nursing facilities, and dialysis centers your crews frequently transport to, and populate destination fields from it. A billing edit that requires both pick-up and drop-off ZIP codes prevents most N756 denials.

Can I just add the address and resend?

Yes, as a corrected claim with resubmission code 7 and the original claim number, unless the payer rejected it as unprocessable, in which case send a new claim.
