What N685 means

Dental claims for crowns, inlays, bridges, and dentures carry an indicator that tells the plan whether the restoration is going in for the first time or replacing an existing one. The plan relies on it because replacements are often subject to frequency limits, such as no new crown on the same tooth within a set number of years, and sometimes need the date of the earlier restoration.

N685 says that indicator was blank, incomplete, or inconsistent with the rest of the claim or the plan’s records, so the plan couldn’t apply its rules.

Common causes

  • The dental software didn’t populate the indicator for the procedure.
  • The office marked initial placement for a tooth the plan’s records show was restored before.
  • A replacement was reported without the prior placement date the plan requires.
  • The indicator was reported on a procedure that doesn’t use it, or for the wrong tooth.
  • Paper claim fields for replacement information were left blank.

How to fix it

  1. Review the patient’s history for the tooth, including any prior crown, inlay, or prosthesis and its placement date.
  2. Correct the indicator: initial placement or replacement, with the prior date when it’s a replacement.
  3. Confirm the tooth number matches the restoration.
  4. Resubmit a corrected claim following the plan’s correction process.
  5. Attach support, such as radiographs or records of the old restoration, if the plan asks for proof of condition or age.

How to prevent it

Set your dental practice software to require the placement indicator for every prosthetic, crown, and inlay procedure. Record prior restoration dates in the chart at the new-patient exam, including work done elsewhere, so replacement claims can be completed accurately.
