N632 Remark Code (Deactivated): Zero Value in OMFS

N632 meant that under the Official Medical Fee Schedule, the service had a relative value of zero, so no payment was due. X12 deactivated it and suggested CARC W8, which was itself later replaced by P19.

X12 deactivated RARCN632 on July 1, 2014. Payers should no longer use it on new remittances, but it can still appear on older ERAs, corrected claims, and appeals.

What N632 meant

Remark N632 was used in workers’ compensation billing. It explained that the service, under the jurisdiction’s Official Medical Fee Schedule, carried a relative value of zero. With zero value, the fee calculation produced no payment.

What replaced it

X12’s note suggested CARC W8. CO-W8 was a workers’ compensation adjustment code for zero relative value, and it was later replaced by CO-P19 in the property and casualty P-series. Services bundled into another same-day service use CO-P14.

It was active only briefly and appears in older workers’ compensation files. Confirm the billed code and the fee schedule version for the date of service. If a different, valued code better describes the service, rebill; otherwise post the line as a non-payable adjustment.
