N474 Remark Code: Incomplete or Invalid Certification

N474 means the certification was incomplete or invalid. The payer received a certification for the service, but it was missing required information, unsigned or undated, expired, on the wrong form, or inconsistent with the claim.

What N474 means

N474 means the certification exists and reached the payer, but it did not satisfy the requirement. Certifications have rules: who can sign, what they must state, which dates they cover, and sometimes which form must be used. When any of those fall short, the payer returns N474, often with CARC 251.

Where certifications go wrong

Timing

  • The certification period ended before some billed dates.
  • It was signed later than the payer allows.
  • A recertification was due and not obtained.

Content

  • Required statements or elements are missing, such as the reason for need or the expected duration.
  • The services or items listed differ from what was billed.
  • A required prerequisite, like a documented face-to-face encounter, is not referenced.

Authentication and form

  • Missing or unacceptable signature, or signed by a practitioner the payer does not allow.
  • An outdated or wrong version of the payer’s form.

How to fix it

  1. Find out which defect the payer identified.
  2. Compare the certification with the claim, including dates and services.
  3. Get a corrected or new certification from the practitioner, properly dated, on the correct form.
  4. Correct the claim if the billed dates or services were the problem, with resubmission code 7.
  5. Resubmit the certification and request reprocessing; appeal if you believe the original met the requirement.

How to prevent it

Track certification start and end dates for every patient receiving ongoing services, and alert staff before they lapse. Keep current versions of payer certification forms in one place so outdated forms are not used.

What is the most common certification defect?

Timing. A certification that does not cover all billed dates, or that was signed outside the payer's required window, is a frequent cause.

Can the certifying practitioner correct it?

They can issue a new or amended certification, properly dated. Backdating is not acceptable.

What if the certification lists different services than billed?

The certification needs to support what was provided. Get an updated certification if care changed, or correct the claim if it was billed wrong.
