MA88 means the address and/or telephone number of the insured person under the primary payer was missing, incomplete, or invalid. The payer needs it to coordinate benefits, typically when it is paying secondary.

What MA88 means

MA88 is one of a group of Medicare remarks about primary payer details on claims where the receiving payer is secondary. It says the address and/or telephone number for the insured under the primary coverage was missing, incomplete, or invalid. Payers use this information to confirm the other coverage and pursue coordination of benefits, especially under Medicare Secondary Payer rules.

It usually comes with CARC 16.

Common causes

  • The insured’s address was left blank because the policyholder is not the patient and registration only captured the patient’s details.
  • The insured’s address was entered in the wrong fields.
  • A partial address, such as a missing ZIP code, was sent.
  • The billing system did not map the other subscriber’s address to the electronic claim.

How to fix it

  1. Confirm the primary insured and their current address and phone from the patient or the insurance card.
  2. Update the account so the policyholder’s details are stored separately from the patient’s.
  3. Complete the claim fields for the insured’s address (CMS-1500 item 7 or the corresponding 837 loops).
  4. Resubmit the claim with the primary payer’s payment information.

  • Collect full policyholder details, not just the patient’s, at registration whenever the patient is a dependent.
