What MA74 means

MA74 is an alert on a reissued payment. The payer already adjudicated this claim and issued payment, but the check or transfer was lost, damaged, or returned. This remittance replaces it. The amounts are generally the same as the original processing; nothing about the coverage decision changed.

What to do

  1. Look up the original payment for the claim in your system.
  2. Confirm you never deposited it. If the original was voided or returned, post the replacement against the same claim.
  3. Do not double-post. If the original payment was recorded, reverse it before posting the replacement.
  4. Fix the root cause. Update your remittance address or EFT enrollment with the payer if a returned payment triggered the reissue; see provider enrollment issues.

Codes that may appear with MA74

  • CO-45 (Charge exceeds fee schedule/maximum allowable or contracted/legislated fee arrangement.): The original fee schedule adjustment carries over onto the reissued payment.
  • PR-2 (Coinsurance Amount): Patient coinsurance from the original processing is repeated, not newly assessed.
  • MA67 (Alert: Correction to a prior claim.): A correction to the claim itself, which can change amounts, unlike a simple reissue.
  • MA72 (Alert: The patient overpaid you for these assigned services.): Addresses a patient overpayment refund, not payer check replacement.

No. It replaces a payment that never reached you or was returned. Post it against the original claim and void or reverse the original payment record if it was recorded.

Common reasons include an outdated remittance address or bank details. Confirm your payment information with the payer.

What if I did receive the original payment?

Contact the payer. Cashing both would create an overpayment that must be returned.
