MA57 Remark Code: Patient Revoked Religious Care Election

MA57 means the patient submitted a written request to revoke their election for religious non-medical health care services. After the revocation, care billed under that election is not covered.

What MA57 means

A Medicare beneficiary who elected religious non-medical health care can change their mind. MA57 tells you the patient did so in writing. From the revocation date on, the religious non-medical care benefit no longer applies, so RNHCI days billed after that date are not covered under it.

The code is specific to religious non-medical health care institutions and is not about medical claims from other providers.

Common causes

  • The RNHCI billed days after the effective date of the patient’s written revocation.
  • The institution was not informed of the revocation, for example because it was filed with another party.
  • The revocation date on file differs from the institution’s records.

How to fix it

  1. Get the revocation date from the payer and the patient’s written request.
  2. Adjust the claim to include only days before revocation.
  3. Resubmit through the payer’s adjustment process.
  4. Request reprocessing or appeal if the payer’s revocation date is wrong.
  5. Review patient liability before billing for any days after revocation.

MA57 FAQ

How is MA57 different from MA55?

MA57 is a voluntary revocation by the patient in writing. MA55 is an automatic revocation triggered by receiving medical care.

Can the patient's medical care now be covered?

After revoking, the patient can receive medical treatment under their regular Medicare coverage. MA57 only affects the religious non-medical care benefit.

What if the revocation date is wrong?

Ask the payer for the date on record and compare it with the patient's written request. Request reprocessing if the date was applied incorrectly.
