MA29 Remark Code (Deactivated): Provider Name or Address

MA29 meant the provider's name, city, state, or ZIP code on the claim was missing, incomplete, or invalid. X12 deactivated it without naming a successor; active remarks now separate the name (N256), address (N258), and service facility address (N294).

X12 deactivated RARCMA29 on June 2, 2005. Payers should no longer use it on new remittances, but it can still appear on older ERAs, corrected claims, and appeals.

What MA29 meant

Remark MA29 said some part of the provider’s name or location on the claim was missing or did not match: the name, city, state, or ZIP code. Payers use those fields to confirm who is billing and, in some cases, where the service was provided.

What replaced it

X12 did not name a successor. Active remarks now point to the exact field and role: N256 for the billing provider name, N258 for the billing provider address, and N294 for the service facility address.

If you still see MA29

It only exists in older remittances. If a current claim is rejected for a name or address issue, compare the claim with the payer’s enrollment record and the NPPES entry for the NPI. Update whichever is out of date, including enrollment if you moved, then resubmit.

The billing address and ZIP code help the payer match the provider to its enrollment record and, for some payers, determine the pricing locality.

What causes mismatches most often?

A practice moving or opening a location without updating its payer enrollment, a post office box in a field that requires a street address, or an incomplete ZIP code.
