MA19 Remark Code: Medigap Crossover Failed, Bill Directly
MA19 is an alert that the claim was not forwarded to the patient's Medigap insurer because the information you submitted about that insurer was incorrect or invalid. You should verify the information and send the secondary claim directly to the Medigap insurer.

What MA19 means
Medicare can forward claims to Medigap insurers so providers do not have to bill them separately. That only works when the claim identifies the Medigap insurer correctly. MA19 tells you the identifying information you gave was wrong or unusable, so no crossover happened.
The Medicare side of the claim is complete. What is left is the Medigap balance, which is now yours to bill.
What to do
- Check the Medigap details. Compare the patient’s card with the insurer name, identifier, and policy number on your claim.
- Bill the Medigap plan directly. Send a claim with the Medicare adjudication information attached or included.
- Correct your records. Update the patient’s secondary coverage so future claims cross over.
- Hold the patient balance until the Medigap insurer responds.

What information is usually wrong?
Often the Medigap insurer's identifier, the policy number, or the insured's details in the other-insurance fields, such as boxes 9 through 9d on the CMS-1500.
Do I need to resubmit to Medicare?
No. Medicare's payment stands. Send the secondary claim directly to the Medigap insurer with the Medicare payment information.
Will future claims cross over?
Only if you correct the Medigap information in your system. Update it now so the next claim is forwarded automatically.
