MA127 Remark Code (Deactivated): Reserved, Never Defined

MA127 was reserved for future use and never had a defined meaning. X12 deactivated it without assigning a meaning or replacement, so a remittance should not rely on it to explain anything.

X12 deactivated RARCMA127 on June 2, 2005. Payers should no longer use it on new remittances, but it can still appear on older ERAs, corrected claims, and appeals.

What MA127 meant

Nothing, officially. The entire description of MA127 was “Reserved for future use.” It sat in the remark code list as a placeholder and was later deactivated without ever being given a message. That makes it unusual among retired codes, which almost all had a meaning at some point.

What replaced it

There is no replacement, because there was never a meaning to carry forward. If you need more explanation for an adjustment, the CARC on the line, such as CO-16, and any other remark codes present are what matter. Payers that plan to explain a decision separately can use N202.

If you still see MA127

A remittance should never have used it, so an appearance in your data is most likely a mapping error in a payer or vendor system, or a misread of a different code. Check the raw 835 file to confirm the value. If it really is there, contact the payer and ask what message it intended, and meanwhile work the claim based on the adjustment reason code.

N202 (Alert: Additional information/explanation will be sent separately.): Alert that additional information will be sent separately, a useful request when a remark is unclear.
CO-16 (Claim/service lacks information or has submission/billing error(s).): The most common CARC for missing information, which payers pair with real remark codes.

Why would X12 reserve a code?

Code lists sometimes hold a number open for a planned message. If the plan changes, the reserved number can be deactivated without ever being used.

What if MA127 appears in my data?

Treat it as a data error or a payer-specific misuse. Ask the payer what it meant, and rely on the adjustment reason code on the same line.
