M58 Remark Code (Deactivated): Resubmit After Corrections

M58 meant the claim had missing, incomplete, or invalid information and should be resubmitted once corrected. X12 deactivated it without naming a replacement; payers now point to the specific field with a targeted remark, sometimes alongside MA130.

X12 deactivated RARCM58 on February 5, 2005. Payers should no longer use it on new remittances, but it can still appear on older ERAs, corrected claims, and appeals.

What M58 meant

Remark M58 was a broad instruction: something on this claim is missing or wrong, fix it and send it again. It did not say which field. It generally accompanied a missing-information adjustment and meant the claim had not been adjudicated on its merits.

What replaced it

X12 did not name a replacement. Payers now pair CARC CO-16 with remarks that identify the problem field. Medicare and others also use MA130 to say the claim is unprocessable and must be resubmitted as a new claim, and N34 when the wrong claim form or format was used.

If you still see M58

It will only appear in older data. If you inherit an old claim with M58, re-check every required field against the payer’s rules for that period. For current denials, look for the more specific remark code on the same line and correct that element before resubmitting.

Often they did not, which is one reason it was retired. Current remarks name the specific element, such as a date, identifier, or code, so the correction is clear.

Is a claim with this kind of remark appealable?

Usually not. Unprocessable claims are typically corrected and resubmitted as new claims rather than appealed. MA130 says so explicitly.
