CO-W7 Denial Code (Deactivated): Comparable Service Allowed

CO-W7 meant the procedure billed was not listed in the jurisdiction's fee schedule, so the payer made an allowance for a comparable service. X12 deactivated it and its notes direct payers to code P18.

X12 deactivated CARCW7 on July 1, 2014. Payers should no longer use it on new remittances, but it can still appear on older ERAs, corrected claims, and appeals.

What CO-W7 meant

State fee schedules do not list every procedure. When a workers’ compensation bill included a code the schedule did not contain, some carriers priced it by picking a comparable service that was listed. CARC W7 explained that approach and the resulting adjustment.

What replaced it

X12’s note says to use code P18. CO-P18 now reports the same comparable-service allowance. When the payer instead asks for a resubmission with a listed code, it uses CO-P7. Standard reductions to listed amounts use CO-P12.

If you still see CO-W7

It is only on older workers’ compensation remittances. Ask the carrier which comparable service it used and compare it with the actual procedure. If the comparison is reasonable, post the adjustment. If a different listed service fits better, or the state provides a by-report method for unlisted services, request reconsideration with supporting records.

CO-W7 FAQ

How is W7 different from a request to rebill?

With W7 the payer paid, using a comparable listed service to set the amount. With P7 (and the older 220), the payer asks you to resubmit with a code that is in the schedule.

Can I dispute the comparable service chosen?

Yes, if you believe another listed service is a closer match. Explain the clinical similarity and request reconsideration through the state's process.
