CO-D11 Denial Code (Deactivated): Pacemaker Registration

CO-D11 meant the claim lacked a completed pacemaker registration form. X12 deactivated it and named CARC 17, which has also been retired; today the problem is usually reported with CARC 16 and remark M132.

X12 deactivated CARCD11 on October 16, 2003. Payers should no longer use it on new remittances, but it can still appear on older ERAs, corrected claims, and appeals.

What CO-D11 meant

CARC D11 applied to cardiac device claims. Payers that required a pacemaker registration form with implantation claims used this code to deny claims arriving without a completed form.

What replaced it

X12’s note points to code 17, covered on the CO-17 page, but that code was also later deactivated. In current remittances the same problem is typically shown as CO-16 with remark code M132 for a missing pacemaker registration form, or N235 when a form was sent but was incomplete or invalid.

If a payer asks for device registration or device information today, gather the device manufacturer, model, and serial details from the operative record and implant log, complete whatever form or claim fields the payer specifies, and resubmit. Check the payer’s current policy, since device reporting requirements have changed over time.
