CO-67 Denial Code (Deactivated): Lifetime Reserve Days

CO-67 reported lifetime reserve days used on an inpatient stay. X12 deactivated it because that count is carried in the 835's supplemental quantity segment (QTY with qualifier LA) instead of as an adjustment reason.

X12 deactivated CARC67 on October 16, 2003. Payers should no longer use it on new remittances, but it can still appear on older ERAs, corrected claims, and appeals.

What CO-67 meant

CARC 67 was used on hospital inpatient remittances to show how many lifetime reserve days the stay consumed. Medicare beneficiaries have a limited pool of these days that can extend inpatient coverage after regular benefit days are exhausted, and hospitals need the count both for billing the patient’s share and for tracking what remains.

The problem was that a count of days is not really a reason for a dollar adjustment, which is what CARCs are for.

What replaced it

There is no replacement CARC. The official text notes that the value is handled in the QTY segment, with qualifier LA, so the count now travels as a supplemental quantity on the claim. Dollar amounts tied to those days are reported with ordinary codes such as PR-2 for coinsurance. Related inpatient codes include CO-78 and CO-69.

If you still see CO-67

It will only appear on very old inpatient remittances or in historical conversion data. When reconciling, treat it as information about days used, not as a write-off, and look for the associated coinsurance amount to determine what the patient owes. What are lifetime reserve days?

Under Medicare Part A, a beneficiary has a limited number of extra inpatient days that can be used once the regular benefit period days run out. Each day used is subtracted from that lifetime total, and a daily coinsurance applies.

Where does the lifetime reserve day count appear now?

In the claim-level QTY segment of the 835, identified by the LA qualifier, as the code's own text notes. It is a quantity, not a dollar adjustment.
